My research group is working on making diagrams such as FlowCharts accessible for vision impaired users.

Sina,

I am having problems filling out your survey using jaws. On the cell phone page, it isn't letting me make my selections and then find the continue button.

I'm sorry to hear this. around 100 or more various screen reader users have taken the survey so far, and they haven't had issues, so
let's try to narrow yours down so we can definitely solve it.

I may have had a similar problem although mine was on the tablets
page.  It wouldn't let me go beyond the page until I selected not
applicable from the how long have you used a tablet...  My guess is it
might be aproblem on all such pages because the question above says if
you don't use said device leave it blank yet the follow-up question
requiresa selection.  This is just a suspician however.

I am running jaws 13.0.852 and IE 8.0

Operating system windows 7 64-bit.
Admittedly my system sometimes just acts funky, so I will give it another try soon.

Aha, I see the issue, if that's indeed the case?

Yes we were hesitant to give radio buttons  a default value, and unfortunately, I can't link the answer of radio button to another,
without changing pages on the user, which I didn't want to do more than necessary.

Sorry about this. I most likely will not use Google docs again for a survey because of the accessibility issues. Let me know if I
can help at all?

I found the survey to be quite accessible, after getting error about not 
filling out a required option it was quite obvious to me I should check the 
not applicable radio button. No matter how simple you want to make 
something, someone is always going to wine. Stick with googledocs, I have 
seen a lot worse.

Wow, you have no idea how happy that makes me. a colleague and I spent some time trying to get Google Forms to be accessible. I
shall let him know it was worth it.

More importantly, thank you so much for your time, your answers, and your spreading this around. I promise we'll actually use this
data. You have my word. We already are, in fact ... we just submitted a paper with the design/implementation of a system that
implements a few of the features dictated by the results of the survey.
